HOW DO I FILL THE FORM OUT?

WHAT IS THIS FORM?

If this is your first time at RJ Physical Therapy group, we do require a New Patient Form to be filled out prior to your first appointment with us. This just helps us know who you are and how we can help you! This can be done either in the office, or if you would like to speed up the process, you can download, print the form and fill it out at home in order to make sure you are seen as soon as possible once you get here.

CAN YOU FILL OUT THE FORM ONLINE?

No, we do not offer an online service to fill out the form online. This form does contain sensitive personal information that should not be shared. This is why we only offer a downloadable PDF file for print to ensure the security of our patients personal information.
